Scaly lichen is quite an unpleasant disease, especially if there is psoriasis on the face. Of course, seborrheic psoriasis on the face is a rather rare and atypical phenomenon, but this does not make it easier for patients, since the disease is chronic and will periodically occur again and again. The maximum that patients can count on is remission in several years, since the disease is incurable for modern medicine. Nevertheless, with psoriasis on the face, treatment will be able to significantly reduce the manifestations of the disease and significantly increase the period of remission.

Symptoms of psoriasis on the face

As already mentioned, seborrheic squamous lichen is an atypical form of the disease that usually occurs against the background of a genetic predisposition and can manifest itself at any time if there is a failure in lipid metabolism, the endocrine system or immunity. In psoriasis on the face, the symptoms are almost the same as the manifestations of scaly lichen on the trunk, and look like inflamed, reddish, rounded plaques that are raised above the skin. In addition to the aesthetic side associated with plaque as such, patients are often worried about itching and severe peeling in the affected areas of the skin.

Of all the areas on the head, psoriasis is the most common on the forehead. Less commonly, scaly lichen settles around the eyes and on the eyelids, in the area of \u200b\u200bthe nasolabial folds and eyebrows. Lip rims are often involved in the pathological process (lip psoriasis). Psoriasis in the mouth occurs in 3% of cases of scalp psoriasis. Ointments for psoriasis on the face and other local agents are the basis of treatment at the initial stage of the disease. In a mild form, it can be a salicylic ointment, and in a severe form, it can be glucocorticoid. As for the face cream for psoriasis, it must be natural and intended for dry or very dry skin (for dry skin / for extra dry skin). At the same time, the effect of such creams will be the higher, the fewer flavors and dyes they contain, and the thicker their consistency. It is desirable that the basis is glycerin, solid oil or petroleum jelly. Equally important is proper skin care in areas affected by lichen scaly. So after washing, it is strictly forbidden to wipe off with a towel so as not to damage the cover. You can gently pat your skin dry with a soft towel, but no more.

In addition to topical preparations, the doctor may prescribe the patient a mild sedative in small doses, since one of the causes of exacerbation of psoriasis is stress. Syphilis in the mouth is a common disease of the modern generation, which ignores the rules of healthy sexual contacts, regular medical examinations. In some cases, it is enough just to use an object of an infected person - a toothbrush, fork, toothpick, etc. Unfortunately, the disease can only be treated with early stages, which are not always diagnosed in a timely manner.

In the article we will consider the causes of infection with treponema, the main symptoms at each stage of the disease, and methods of effective therapy.

Many patients do not have a single example of syphilis infection in their lives. And, nevertheless, this does not make the disease more rare or less dangerous, since absolutely everyone can be infected with it at any age and place.

In the mouth, the disease can manifest itself for several reasons.



Disease stages and characteristic symptoms

Many patients are interested in what syphilis looks like in the mouth (photo below). Just like a regular one, it goes through 3 stages and has characteristic visual manifestations. The first phase is characterized by the appearance of a specific hard chancre (painless ulcers), in the second phase they disappear and are replaced by roseola (a rash on the mucous membrane), at the end, in the third phase, a gum (node) appears, which changes the structure of soft tissues and even bones.

Let's consider each of the stages in more detail.

First phase

The first phase - the chancre develops. In the area where the pathogen has entered the oral cavity, a rounded, reddish and sufficiently hard sore appears that does not cause discomfort to the patient. Chancre can appear in any area of \u200b\u200bpenetration of the triponeme - the inner part of the cheeks or lips, tongue, palate, tonsils and even the gums. The diameter of the ulcer does not exceed 20 mm, and the edges are slightly above the center. In some cases, not one, but several nearby chancre appears.

If we are talking about syphilis of the tongue, then the chancre may resemble a deep groove hidden in the fold of the mucous membrane.

Subjective sensations of the patient at this stage: induration in the oral cavity of unknown origin, enlargement of nearby lymph nodes.

Second phase

The second phase is the attachment of papules, stomatitis and other formations. At the second stage, the chancre disappears, and various rashes on the body and mucous surface come to replace. After 7-9 days, reddish roseola forms in the oral cavity, which combine into the affected areas and increase. As a rule, this process flows into acute diffuse stomatitis.

Roseols do not cause discomfort to the patient, so it is also difficult to notice them in a timely manner.

The spots are located on the palate and tongue, the inner rim of the lips. They contain a high concentration of the pathogen inside. If during this period you do not start taking antibiotics, then the process of relapse begins. The spots disappear, then appear again, but already in the form of papules and plaques.

Papules, unlike roseola, are not symmetrically located in the mouth, forming the so-called "patterns". If you try to remove their top layer, then peeling begins at the edges. If located on the tongue, receptors and papillae in the area atrophy.

In some cases, purulent abscesses (pustules) may appear in the second stage. This process is accompanied by a sharp swelling of the lymph nodes of the whole body and hyperthermia.

Stage two symptoms are highly contagious, since the formations contain a high density of the tryponeme. During this period, the patient should be immediately admitted to the hospital and isolated from others.

If effective therapy is not started in the second phase, pathogens will invade the larynx and pharynx, which will cause throat syphilis.

Third phase

The third phase - the appearance of gum, glossitis... The disease in this phase is quite neglected, therefore, new formations - nodules (gummas), abscesses and bumps - join the papules. This process can take about 4-5 months.

At the beginning of the phase, the nodes are seals on the tongue or palate, after the disappearance of which, bulk ulcers remain. When the wound heals, an atrophic scar remains in its place, which greatly deforms the soft tissues and even the jaw bones. In rare cases, a canal appears between the nose and mouth or the mouth and sinuses.

During this period, the patient cannot speak articulately. Diffuse glossitis joins the syphilis of the oral cavity, which provokes the development of connective tissue. The tissue of the tongue thickens, which makes it practically immobile, interfering with food intake and communication.

Pathologies that develop in the third phase are not amenable to treatment or natural recovery. In most cases, the patient is prescribed medical plastic or prosthetics.

Diagnosis of the disease

Initial examination and identification of formations may indicate the possibility of infection of the patient with syphilis. Inflamed areas, roseola, spots, chancres have clearly defined boundaries. When diagnosing such formations, the doctor should refer the patient to a dermatovenerologist, who, in turn, will send the discharge from the cavities for examination. The study is carried out under a microscope, which reveals the content of tryponema in the pathological fluid.

Unfortunately, a positive confirmation can only be obtained 2-3 weeks after the appearance of the primary foci of inflammation. Also, the discharge from the chancre may not contain the pathogen, so the venereologist will try to find them in the lymph nodes.


It is possible to obtain a positive confirmation of syphilis only after 2-3 weeks from the moment the primary foci of inflammation appear.

Serologic tests and procedures can detect syphilis in the mouth at any stage. The Wasserman reaction reveals an increase in the number of antibodies. However, the method does not always show reliable information, as pregnant women and people with autoimmune diseases may have a false positive reaction.

An effective method for detecting the disease can be immunodiagnosis, analysis of punctate from the fluid of the spinal cord, blood test for syphilis. The latter is prescribed to people at risk (health workers, vulnerable groups of the population, pregnant women, workers in children's institutions and catering, donors).

Also, the analysis is required for people who are sexually active, who enter into oral and homosexual acts.

Treatment of the disease and possible complications

Therapy in this case is aimed at suppressing the vital activity of the triponeme. Effective symptomatic treatment is also required, and the sooner, the safer for the patient (the lower the risk of complications and relapses).

As a rule, the doctor prescribes bactericidal medicines of various groups and forms, as well as immunostimulants. Treatment is carried out in several courses, between which long pauses should be maintained.

Symptomatic treatment involves taking antipyretic drugs, the use of local wound healing and tissue regenerating agents, applications, baths, rinses, etc.

At the end of the course of treatment, the doctor conducts a serological blood test for several months.

If therapy is prescribed out of time, then the disease will cover not only the soft tissues of the oral cavity, but also the internal organs and systems, which will entail a general deterioration in the patient's condition.

With each month, the symptoms of syphilis in the tongue (photo below) and other areas of the oral cavity will fade, which patients mistakenly attribute to improvement.

However, after a while, serious complications should be expected:

  • necrosis of soft tissues in the area of \u200b\u200binfection;
  • damage not only to the mucous membrane, but also to the internal organs, bones;
  • disruption of the circulatory and vascular systems, the occurrence of local bleeding;
  • damage to the soft tissues of the face and neck, asymmetry of facial contours;
  • slow destruction of brain cells.

Remember that a complete cure in stages 2 and 3 is almost impossible. Visit your dentist regularly, get tested, have a weekly oral self-examination, and avoid promiscuous sexual intercourse. It is also important to know that the body does not develop immunity to syphilis, which means that re-infection is possible.
