The procedure for obtaining a quota for a surgical VMP in 2018 - a list of documents and stages of registration

The main document for referring residents of Russia to specialized clinics for the provision of high-tech medical care is the corresponding order of the Ministry of Health and Social Development of the Russian Federation.

The quota registration procedure is as follows.

The Ministry of Health distributes "quotas" for treatment in the indicated regional clinics. And each region has the right to send residents only to where the quota was allocated.

The Ministry of Health has approved a commission to consider applications for high-tech medical care. The selection is made based primarily on the medical indicators and the patient's condition, as well as other factors that may affect the course of the disease.

In case of a positive decision, the patient receives a coupon for the provision of high-tech medical care. The coupon must be accompanied by a list of diagnostic tests that are necessary for hospitalization in the recommended medical institution. Treatment according to the "quota" VMP

High-tech medical care (HMP) - This is medical care using high technologies for the treatment of complex diseases. It includes both medical and diagnostic services that are performed in a specialized hospital. Each year, the Ministry of Health approves the list of medical institutions and the list of diseases for which VMP is provided. The program for the provision of HMP to Russian citizens was launched back in 1994. Then the Russian government issued a special decree to allocate a certain amount from the federal budget for the provision of high-tech medical care for each individual patient. This is called the "quota". Currently, the Russian Ministry of Health has abandoned such a concept as "quotas for high-tech medical care." Now it is customary to talk about the volume of the UMP, since quotas do not sound entirely correct in relation to people with serious illnesses. Previously, VMP funding came only from the federal budget, but starting in 2014, funds began to be attracted from the compulsory health insurance (MHI) funds. Everything related to high-tech care is under the jurisdiction of the Russian Ministry of Health. It is this body that annually determines the number of "quotas" that each medical institution will receive. Medical institutions cannot exceed this plan and treat more patients than the Ministry of Health planned.

Absolutely every citizen of the Russian Federation has the right to receive free high-tech medical care. The main reason for obtaining a VMP is a medical indication.

How to get a VMP quota?

The coupon for the provision of VMP is issued by the regional ministry or the health department at the place of registration of the patient. For this, the patient must provide a referral from his attending physician with an established diagnosis and the results of the necessary examinations. On the basis of these documents, the specialist of the VMP department in the ministry or department himself determines which medical institution to send a patient to for treatment.

If the patient already knows which of the clinics he wants to undergo treatment, then he first needs to contact this clinic, provided that this clinic has been allocated "quotas" for the VMP, and receive a specially issued conclusion that he is shown performing a high-tech medical care in this hospital. With this conclusion, the patient applies to the regional ministry or department of health, where they issue a voucher for a high-tech medical device to the specified clinic.

Despite the fact that the rules prescribe the need for a patient to personally contact the regional health department, in practice this is not always required. Therefore, if a patient from a distant region, for example, from Siberia, comes to Moscow to the clinic of his choice, and the doctors give him a conclusion that high-tech medical care is indicated for him, the patient does not need to go back to his region and take this conclusion personally. The clinic's specialists themselves will send an electronic copy of the conclusion to the regional health authority and receive a response about the issuance of a coupon for the provision of high-tech medical care. It usually takes 1-2 days.

The procedure for obtaining a VMP quota for treatment at the KKMH Clinic

Step 1.

Personally

The patient personally contacts us at the Clinic with the results of the examinations he has, he is consulted by the Clinic's doctor and establishes that the treatment of the patient's disease can be performed using the UMP. After that, a conclusion is issued on the need and possibility of such treatment.

Remotely

The patient is in another region and has the results of tests and examinations on hand. He contacts our Clinic remotely - by phone, e-mail, through the online registration form, and sends us copies of his examinations (electronically or by fax). Doctors of the Clinic of Coloproctology and Minimally Invasive Surgery review the documents received and make a preliminary conclusion about whether the patient is indicated for treatment with the use of high-tech medical care. If the doctor of our clinic decides that the UMP is shown to the patient, then a conclusion is drawn up on the official form of the Clinic.
Step 2.

On my own

The patient takes the received opinion to the regional ministry or health department at the place of registration. The regional authority draws up a voucher for the VMP for treatment in our Clinic and sends it to us electronically. The process of issuing a coupon takes 1-2 days.

The specialists of our Clinic send an opinion on the need to provide a VMP to the regional department at the place of registration of the patient and in return receive a coupon for the VMP, which gives the right to treat this patient.

What is included in the VMP "quota"?

The referral to the provision of high-tech medical care, which is issued to a patient, "covers" all treatment in a specialized medical center, including being in the hospital during hospitalization, medications and consumables provided for by the standards. All this is included in the cost of providing high-tech medical care, which means it is free for the patient. The examination required to establish the diagnosis and prepare for the operation is usually not covered by the "quota", since according to the orders for the UMP, the patient must be admitted for treatment already fully examined at the place of residence.

The legislative framework

Medical care provided to the population is primary health care, specialized, emergency and palliative. At the same time, specialized includes high-tech medical care.

What is VMP

VMP is a medical care that is provided using the latest developments, technologies and equipment. It is used in cases of treatment of complex and serious diseases by specialists with the necessary qualifications.

A medical institution carrying out treatment with the help of a VMP must have a license for this type of activity.

A complete list of HMP types and quotas is indicated in Order No. 916n of the Ministry of Health of the Russian Federation.

The primary decision on the need to use the VMP is made in the clinic or hospital.
Then the documents are considered at the regional level within 10 days, where the final decision is made.

If the outcome is positive, the documents are sent directly to the institution where the treatment will take place.

Within the next 10 days, with an electronic appeal and within 3 days with a personal consultation, the documents are considered by the clinic's commission.

It may take several months from the date of submission of documents to hospitalization. This is due to the queue, the urgency of treatment and the availability of vacancies.

Sometimes a different abbreviation is used to denote high-tech medical care - HTP.

Help with the CHI system

The provision of high-tech medical care is regulated by by order of the Ministry of Health of Russia dated December 10, 2013 No. 916n"On the list of types of high-tech medical care" and decree of the Government of the Russian Federation of December 19, 2016 No. 1403"On the Program of State Guarantees of Free Provision of Medical Care to Citizens for 2017 and for the Planning Period of 2018 and 2019".

The decree is adopted by the prime minister at the end of each year and contains a detailed list of diseases and conditions, types of high-tech medical care, indicating the methods of treatment for which help is free.

Next to each type of high-tech assistance is the standard of financial costs per unit volume. In simple words - this standard is the estimated cost of a procedure, operation, treatment. In the list of high-tech assistance (HTA), the overwhelming number of procedures is estimated at 50-100-150-200 thousand rubles.

The same decree defines the average per capita standards for treatment, for the current year - 2018, and planned for 2019 and 2020:

To some extent, they can be adjusted (we hope to increase) territorial programs, but one should not hope for a large increase.

Taking a look at these amounts, it becomes clear that the case when the patient was provided with an UMP under the compulsory medical insurance can be considered a great success.

Until 2014, VMP was financed exclusively from the federal budget. Later, changes were made, as a result of which funds from the system of compulsory health insurance funds were added to the existing provision.

On the VMP from the basic program, money comes in the form of a part of subventions to, and on the VMP, which is not related to the state program, funds are credited from federal institutions directly. In some cases, the regional budget also participates in financing.

Any Russian citizen has the opportunity to receive high-tech assistance free of charge or on a quota basis (both an adult and a child). But for this, there must be a medical indication.

In some cases, after reviewing the documents, the patient may be denied the use of a VMP.
The reason may be, for example, high risk or ineffective treatment. In this case, alternative methods will be proposed.

If a citizen wants to appeal the decision, he needs to write a complaint to the local authorities of Roszdravnadzor.

With some diseases, there is no time to prepare documents, since help is needed immediately.

In such situations, the citizen receives the necessary HMP, but it is carried out under the compulsory medical insurance only for one area (vital procedure, operation), not including full medical care.

According to research, for 2015-2016. Most of all, VMP is in demand in the treatment of coronary heart disease, arthroplasty, operations for oncology, glaucoma and the installation of a pacemaker.

What types are included in the list of CHI

Under the compulsory health insurance program, the types of high-tech medical care provided are classified by region. The list of diseases and methods of their treatment with the use of VMP is given in the decree.

In total, the decree lists 1435 types of VMP. Some of them:

microsurgery;
reconstructive plastic, restorative operations;
surgical and combined treatment of neoplasms;
surgical organ-preserving treatment;
radiation therapy (including remote, interstitial, radionuclide, etc.);
laser and intensive therapy;
nursing newborns;
